PublicationArticle A clinical study of the interrelationship between sinus disease and lower respiratory tract infections(1980) H.C. Samant; B.K. Samanta; M.K. AgarwalThe present study was conducted on 147 patients in two groups, the first group comprising 61 patients with primarily lower respiratory tract infections, such as chronic bronchitis, bronchiectasis, bronchial asthma, lung abscess and pulmonary tuberculosis, and the second group consisting of 86 patients with predominantly naso-sinal involvement, including 70 cases of chronic sinusitis and 16 cases of allergic rhinitis. In the first group, the maximum number of cases (32.7%) were in the age group between 31 and 40 yr, while in the second group the highest age incidence (38.3% cases) was in the age group of 21-30 yr. Males predominated females in both the groups. Both the groups of patients showed presence of anaemia, a factor responsible for the causation and chronicity of sinorespiratory infections. Chronic bronchitis and bronchial asthma were common in both the groups. Sinusitis was found in 28 patients out of 61 cases of lower respiratory diseases. The incidence of sinusitis in bronchial asthma was found to be 57%. Maxillary sinus was commonly involved in 892.% of the cases. In the second group of patients with naso-sinal infection, only 12 cases (13.9%) revealed lower respiratory diseases, such as chronic bronchitis, bronchial asthma and tropical pulmonary eosinophilia. The presence of respiratory symptoms, such as cough and expectoration, dyspnoea and pain in the chest, in cases of naso-sinal infection, should suggest concomitant lower respiratory tract involvement.PublicationEditorial A study of otorhinolaryngological manifestations in patients of chronic renal failure(1997) M.K. AgarwalThe present study was carried out on 50 patients of Chronic renal failure (CRF), Majority of CRF patients had high frequency hearing loss, but inner ear changes were found to be irreversible even after dialysis. All cases had cochlear lesion. No relationship could be established between the level of blood urea, creatinine, serum potassium, serum calcium and blood pressure with the severity of the impaired hearing. A direct correlation was seen between increasing hyponatraemia and impaired hearing. No direct relationship could be made between blood pressure and epistaxis, but all the cases had raised blood urea. © 1997 Association of Otolaryngologists of India.PublicationArticle Aspergillosis of the maxillary sinus(Springer India, 1998) M.K. Agarwal; P.K. ShuklaA rare case of aspergillosis of the maxillary sinus is reported. Surgical excision including the lining of the infected sinus is the preferred method of treatment. GuptaThe present study was carried out on 75 patients suffering from chronic otitis media and 25 healthy age and sex matched controls. The T. rosette percentage were depressed in malnourished pretreatment test cases and showed depressed cell mediated immunity (CMI), but once malnutrition was treated the CMI was improved. Anergy to challenge dose of 2:4 DNCB was not observed in any groups. Weak response was observed in 4% of controls, 18.7% of chronic otitis media patients and 5.7% of post treatment cases. Serum IgG, IgA and IgM levels were elevated in chronic otitis media patients from controls but once the infections were eliminated by treatment their values became insignificant (p 0.05) from controls. AgarwalEpithelial tissue isoantigens A,B and H were studied in 182 patients with oral lesions. These patients included 24 patients with premalignant leukoplakic dysplasias and 158 with oral squamous cell carcinomas of various sites in the oral cavity. Loss of isoantigen was seen in 33.33% of patients with leukoplakias and 36.23% of those with carcinomas. The test was found to be negative in all patients showing regional lymph node or distant metastases. The present study indicates that the test can be usefully employed in the evoluation of oral malignancy, its early immunologic diagnosis and as a guide to prognosis in followup studies.PublicationArticle Extramedullary plasmacytoma of the larynx(1979) M.K. Gupta; S. GuptaTwo cases of hydatid cyst are reported, one in the right parotid region involving the deep lobe and the other in the superficial subcutaneous tissue of the sub-mental region. Cystic swellings, where no definite preoperative diagnosis can be arrived at, call for a high suspicion rate for hydatid disease and, even in the nonendemic areas, Casoni's test and complement fixation tests should be carried out. © 1978 Association of Otolaryngologists of India.PublicationArticle Immunological evaluation of children undergoing tonsillectomy(1979) D. Gogol; O.P. Gupta; M.K. Agarwal; R.M. GuptaThis study was carried out on 80 patients undergoing tonsillectomy for chronic tonsillitis. Twenty healthy school children in the same age group were investigated for control values. Ten children were also available for evaluation of any immunological change after tonsillectomy. No significant variation was seen in the serum IgG, IgA, and IgM levels before and after tonsillectomy. Peripheral absolute lymphocyte count was higher in the pre-operative cases as compared to the control group, and this count became near normal following tonsillectomy. Skin tests with recall antigens PPD and Candida and contact sensitizing agent 2:4 DNCB showed an improved cell mediated immune response following tonsillectomy.PublicationArticle Multiple myeloma invading the larynx(1981) M.K. Samant; O.P. Gupta; B.M. SharmaThe present study was carried out on 185 patients. Aspergilli or Candida were isolated in 93.5% of cases. A.niger was found to be the most common fungus causing otomycosis (44.3 %). Mixed infection with more than one fungus was infrequent. Proteus species, staphylococcus aureus and E.coli were the commonest pathogenic bacteria isolated from 60.8% cases of otomycosis associated with discharge from the ears. An experimental study was also made for the production of otomycosis in 20 human volunteers. Better results were obtained when debris was used to inoculate the healthy ear. In 6 cases (30.7%), the healthy ear could be infected successfully. © 1980 Association of Otolaryngologists of India.PublicationArticle Mycosis fungoides of the larynx(1982) M.K. Agarwal; S. Gupta; O.P. GuptaA rare case of mycosis fungoides involving almost the right half of the larynx in a 55-year-old male is being reported perhaps for the first time in the literature. Clinically the case appeared to be laryngeal cancer and diagnosis was established only by histopathological examination.
