A Probable Case of True Hermaphrodite With 46,XXq/ XY Mosaicism
(Georg Thieme Verlag, 2024) Shail Khanna; S.N. Maitra; Sushila Khanna; T. Sharma
A married women of 35 years age with primary amenorrhoea and poorly developed secondary sex characters but having a frankly feminine external genitalia was found to be a probable case of true hermaphrodite. There was a normal sized testis with epididymis and vasdeferens within the left swollen labia majora. At the position of the overies two greyish white bodies were present, which looked very much like ovaries, but small in size and inter-connected by a band of fibrous tissue. In buccal as well as vaginal smears a very low percentage of cells had a sex-chromation body. The size of the sex chromation was howeevr, much reduced. The G-band analysis of chromosomes, made from leucocyte cultures, revealed a 46, XXq-|46, XY sex chromosome mosaicism with predominance of 46, XY type of cells. © 2024 Georg Thieme Verlag. Cell surface antigens in squamous cell carcinoma of the oral cavity
    (1986) S.N. Das; N.N. Khanna; Sushila Khanna; S. Gupta
The ABH isoantigen was investigated in 80 patients with squamous cell carcinoma of the oral cavity and in‐their 46 affected lymph nodes. Isoantigen deletion shows a characteristic pattern of benign to malignant transformation of oral cancer. Total loss of isoantigen was found in anaplastic tumours, in metastatic lymph nodes and in the majority of advanced stage tumours. Total loss of isoantigen in metastatic lymph nodes indicates a frequent discharge of isoantigen‐negative metastatic cells from the primary tumours to the lymph nodes and the regional organs. It seems that with the recent techniques a prospective observation of ABH isoantigen added to other pathological and clinical findings could be of prognostic significance in patients with squamous cell carcinoma of the oral cavity. Cellular Immune Status In Patients With Cancer Of The Oral Cavity
(Georg Thieme Verlag, 2024) S.N. Das; N.N. Khanna; Sushila Khanna
Cellular Immune Status was observed in 70 patients of squamous cell carcinoma of the oral cavity by in vitzo observation of absolute lymphocyte counts, T-cell counts and lymphocytes blastogenesis to PHA. The results were compared with 40 age matched normal controls. Absolute lymphooy te counts, absolute T-cell, T-rosette forming cells and blastogenic indax was significantly lower in oral cancer (P <.001) as compared to normal controls and further gradual decrease with increasing clinical stage, Well differentiated tumours were associated with higher T-cell counts and blastogenic index than the poorly differentiated tumours. The observations implies that the cellular immune showel a Tesponse is invariably impaired in oral cancer patients and is related to the malignant process in the host. The significant depression of absolute lymphocyte counts, T-cell counts and blastogenic index suggest that the existence of tumours mass itself may play an active role in subverting the host immune competence. © 2024 Georg Thieme Verlag. Characterization and histopathological correlation of cytosol proteins of benign and malignant breast tumors
(1991) Hari D. Khanna; Debasis Das; Jagadish N. Kashyap; Saroj Gupta; Sushila Khanna
Significant differences in cytosol protein level exist between normal/benign and cancerous breast tissues. There is a positive correlation between the cytosol protein level and histological grade of carcinoma. Well‐differentiated carcinoma have a lower value of cytosol protein than poorly differentiated carcinoma. In slab gel electrophoregrams, the total numbers of bands are almost identical in normal, benign, and malignant conditions. In addition, 37 Kd protein band is consistently present in malignant cases and always absent in normal or benign cases. More extensive biophysical examination of this band may provide further insight into the protein alterations in cancer cells at the molecular level. Copyright © 1991 Wiley‐Liss, Inc., A Wiley Company

Fine‐needle aspiration cytology of abdominal masses
(1990) Ajay Kumar Khanna; Mahendra Kumar Misra; Anuradha Khanna; Vijay Kumar Misra; Sushila Khanna
An investigation of the role of blind fine‐needle aspiration cytology (FNAC) in the assessment of palpable abdominal masses was carried out on 196 patients: 124 hepatic, 30 retroperitoneal, and 42 other masses. All of the smears were stained either by Papanicolaou stain or by hematoxylin and eosin stain. The results of FNAC were confirmed by further investigations in all cases. FNAC correctly diagnosed 166 (84.6%) cases. Twenty (10.2%) reports were false negative, and 10 (5.1%) smears were unsatisfactory for any diagnosis. Of 124 hepatic masses, the correct diagnosis was obtained in 106 (85.4%), false‐negative reports in 14 (11.2%), and unsatisfactory smears in 4 (3.2%). There were no false‐positive reports. For all of the lesions, the sensitivity of FNAC was 87.8%; specificity, 100%; positive predictive value, 100%; and negative predictive value, 52.4%. Five (2.5%) patients had considerable pain after the procedure. Copyright © 1990 Wiley‐Liss, Inc., A Wiley Company

Germ cell tumors in Indian children
(1988) Sushila Khanna; N.C. Arya; I.M. Gupta; Saroj Gupta; G.D. Singhal
The present paper is a retrospective study of one hundred and thirty five germ cell tumors seen in children at the University Hospital, Varanasi during a period of 21 years. These tumors constituted 7.5% of all solid tumors in the pediatric age group. Histologically 89 tumors (65.9%) were benign and 46 (34.1%) were malignant. Eighty percent of ovarian and all the testicular germ cell tumors were malignant. Thirty per cent of the tumors were located in the sacrococcyx. The ovaries were the next common site. There were 80 girls, 53 boys, and 2 cases in which the sex was not recorded. The predominance amongst girls was mainly due to the more frequent involvement of ovaries as compared to the testes. Twenty nine of the children were in the first year of life. The factors probably responsible for the higher incidence of malignancy, the differences in anatomic location, and the sex distribution in comparison to other reports have been examined and discussed. In vivo and in vitro observation of cellular immune parameters in squamous cell carcinoma of the oral cavity and its correlation with tumor load and prognosis
(Informa Healthcare, 1986) S.N. Das; N.N. Khanna; Sushila Khanna
In vivo and in vitro observations of cellular immune response in 70 patients with squamous cell cancer of the oral cavity and in 40 age-matched normal controls, were made using delayed hypersensitivity responses to DNCB, PPD, and Candida albicans extract (Dermatophytin (0) absolute lymphocyte counts, absolute T-cell numbers, and PHA-induced lymphocyte blastogenesis reactions as parameters. The results were correlated with clinical stage, tumor size, lymph node involvement, tumor differentiation, lymphoreticular responses, and outcome during a one-year follow-up period. A significant degree of impairment of both in vivo and in vitro parameters was found in oral cancer patients compared to normal control. The impairment was more prominent in advanced stages. Lymph node involvement was associated with impaired dermal hypersensitivity to recall antigens as well as a reduced T-cell population and blastogenic response. Only delayed hypersensitivity response to DNCB, PPD, and Candida showed a correlation with histologic features such as tumor differentiation and lymphoreticular response. Although absolute lymphocyte counts and T-cell population were reduced in the primary stage of the disease, the functional capacity of isolated lymphocytes to undergo blast formation was retained. PHA-induced lymphocyte blastogenesis showed a significant impairment only when the tumor was well established and disseminated beyond its local confines. Delayed hypersensitivity responses to DNCB, higher T-cell counts, and blastogenic indices were associated with recurrence-free survival. Immunologic parameters provide prognostic information beyond the clinical stage of the disease. Therefore, it seems that a multiparametric in vivo and in vitro observation of cellular immune response may be useful as an indicator of clinical course and prognosis of patients with squamous cell cancer of the oral cavity. © 1986 Informa UK Ltd All rights reserved: reproduction in whole or part not permitted.

Placental morphometric and morphologic alterations in maternal undernutrition
    (1976) L.S. Murthy; K.N. Agarwal; Sushila Khanna
Placentas, mothers, and neonates belonging to different socioeconomic groups were examined for morphometric and gross morphologic changes to assess the effect of maternal undernutrition. The mean maternal caloric intakes for socioeconomic Groups I, II, and III + IV are 2,919, 2,420, and 1,589 calories per day, respectively, and differences were significant (P < 0.001). The mean protein intakes were respectively 76.1, 67.3, and 52.0 Gm. per day for socioeconomic Groups I, II, and III + IV, respectively (p < 0.001). The birth weight, placental weight, volume, surface area, and number of cotyledon showed significantly decreasing trends (P < 0.001) with the fall in socioeconomic group and maternal dietary intakes. Placentas of socioeconomic Group I had significantly higher incidence of well-defined cotyledons and central cord attachment. In lower socioeconomic groups and small-for-dates placentas there was significantly higher incidence of hemorrhages. There were no changes in incidences of placental infarcts, calcifications, and degeneration in various socioeconomic groups. © 1976.

Preoperative chemotherapy for fungating breast cancer
(1987) Anand Kumar; L.L. Shah; Sushila Khanna; N.N. Khanna
Thirty cases of fungating breast cancer (stages III and IV) were treated preoperatively by combination chemotherapy using cyclophosphamide (Endoxan), methotrexate, 5‐fluorouracil, and prednisone. Mastectomy was performed after 2 cycles of preoperative chemotherapy followed by 6 more cycles postoperatively. Of 30 patients, 25 (83.3%) were made operable and had better disease‐free survival. Preoperative chemotherapy has an advantage as regards improvement in quality and longevity without much toxicity, and provides a better palliation. Copyright © 1987 Wiley‐Liss, Inc., A Wiley Company

Primary tumours and tumour‐like conditions of the mandible
    (1981) Sushila Khanna; N.N. Khanna; O.P. Gupta; F.M. Tripathi; B. Sanyal; A.B. Srivastava
Reports on comprehensive mandibular tumours are very rare. The present article describes a total of 81 primary tumour and tumour‐like lesions of the mandible seen at the University Hospital, Banaras Hindu University, during last 16 years. In the mandible the ratio of odonto‐genic to nonodontogenic tumors is 1.6:1. Ameloblastomas constitute 56% of the odontogenic tumors. Among the benign mesenchymal tumours of the jaw 58% are neoplastic and 42% are nonneoplastic in which cementoostoid tissue is laid down secondary to inflammatory process. Over all 40 cases are benign 28 potentially malignant (ameloblastoma) and 13 are malignant lesions. Among the malignant mandibular tumours, bone tumours are more common than the soft tissue sarcomas. Microscopically mandibular osteosarcomas are better differentiated than similar tumours in the long bones. Local, segmental, or radical surgery seems to be the treatment of choice in the mandibular tumours. Copyright © 1981 Wiley‐Liss, Inc., A Wiley Company

Sarcomas of the breast: Homogenous or heterogenous?
(1981) Sushila Khanna; Saroj Gupta; N.N. Khanna
Eighteen cases of sarcomas of the breast are reported. Cases could be divided into two groups histogenetically. Group A, malignant cystosarcoma phylloides (14 cases); and Group B, pure sarcomas (4 cases). The malignant cystosarcoma phylloides occurred in the fourth decade, had longer duration of illness invariably followed by sudden increase in the size of the tumor, had a striking mucoid and slimy look grossly, and showed discernable epithelial elements histologically. In the event of their recurrence the ductal elements were not seen microscopically. Contrarily, Group B cases presented in the sixth decade were rapidly growing from the beginning, and grossly were fleshy and haemorrhagic. Microscopically these did not reveal any epithelial component. There was poor correlation between cytologic malignancy and the biologic behavior in Group A. Mode of treatment seems more important in determining the subsequent course of the disease. Simple mastectomy with follow‐up appears to be adequate in the majority of cases. Serum immunoglobulins in squamous cell carcinoma of the oral cavity
    (1982) N.N. Khanna; S.N. Das; Sushila Khanna
Serum immunoglobulins (IgG, IgA, and IgM) have been studied in 70 patients with squamous cell carcinoma of oral cavity and in 40 age‐matched normal controls. The results were correlated with clinical stage, tumor size, lymph node status, tumor differentiation, and treatment modalities. There was a significant rise in serum IgM and IgA in oral cancer patients as compared to controls. It was further raised in advanced clinical stage, but tumor size and histological differentiation and treatment modalities (surgery and chemotherapy) do not seem to affect serum immunoglobulin levels. The observations implies that the extension and dissemination of tumor mass is the important factor for the change in serum immunoglobulin level in squamous cell carcinoma of the oral cavity. Spectrum of breast disease in young females : A retrospective study of 1315 patients
(1998) Rahul Khanna; Sushila Khanna; Sunanda Chaturvedi; N.C. Arya
A total of 1315 breast lesions seen in women up to 40 years of age were registered in the files of Pathology Department. Out of these 807 were benign and 508 malignant lesions. Breast carcinoma was very rare up to second decade of life but it's incidence progressively increased with age. Among benign tumours fibroadenomas were the commonest tumours. This study shows a higher incidence of inflammatory diseases and malignant tumours in young females as compared to the reports from the West.
