    Introduction of Lower Limb Edema

    Lower limb edema is a multifactorial condition affecting a large number of patients and generating considerable burden on the healthcare system. Patients with lower limb swelling may suffer from functional as well as cosmetic compromises requiring lifelong treatment and psychological support. The pathophysiology of edema is mainly based on disturbances in the microcirculation. For the diagnosis, we have to consider whether the swelling is acute or chronic, localized or generalized, congenital or acquired, symmetric or asymmetric. Asymmetry is usually present in chronic edema which arises from venous or lymphatic disease, whereas symmetric lower limb swelling is generally suggestive of systemic or central causes, such as heart failure and nephrotic syndrome. In systemic disorders, medications of the underlying pathology are central in the management while localized underlying causes such as chronic venous disorder, lymphedema, deep vein thrombosis, cellulitis, and trauma may need intervention and meticulous follow-up from time to time. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Singapore Pte Ltd. 2022.

    Minimally invasive techniques as adjuncts in low-versus high-lying retained rectal foreign bodies of autoerotic nature in young men: a tailored management algorithm with two contrasting case reports from India

    Retained rectal foreign bodies (RFBs) of an autoerotic nature represent an emerging and rare surgical emergency, posing a sensitive challenge for surgeons. RFBs exhibit a wide range of presentations and require varied management approaches, with the choice of treatment modality differing from case to case. Recently, minimally invasive techniques have been employed for the retrieval of RFBs. In 2021, the World Society of Emergency Surgery and the American Association for the Surgery of Trauma released guidelines on anorectal emergencies, highlighting the usefulness of these techniques as adjunctive tools for both diagnosis and ruling out associated complications. In this report, we describe two noteworthy cases of men who presented to the trauma emergency department with foreign bodies lodged in their rectums. We also highlight the potential role of minimally invasive techniques within a “step-up” approach for the management of retained RFBs. © 2024 The Korean Society of Traumatology.

    Overview of Management in Lower Limb Edema

    Over the years, many recent advances for diagnosis as well as treatment of lower limb edema have evolved. The current diagnostic tool indocyanine green lymphography (ICG-LG) can detect dermal lymph backflow in asymptomatic legs even at stage 0. At symptomatic stage ≥1, ultrasonography, magnetic resonance imaging-lymphography/computed tomography-lymphography (MRI-LG/CT-LG), and lymphoscintigraphy are also useful. Management includes modification of lifestyle and decompressive physiotherapy, which are mainstay of treatment. Medical management and surgical intervention are considered when the former fails and have limited role and are mostly optional some new therapies are also under trial which may prove to be of great benefit in the future. The goal of therapy is to restore function, reduce physical and psychological suffering, and prevent the development of abnormalities. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Singapore Pte Ltd. 2022.
